Periodic Health Assessment 

Electronic Periodic Health Assessment Process 

To complete your PHA at U.S. Naval Hospital Yokosuka, perform the following steps: 

**Ship commands must see their medical section for PHAs** 

Step 1. Check your Individual Medial Readiness Report 

You’ll need a CAC card reader to access your IMR Report. 

Navy: https://www.bol.navy.mil/ 

Marines: Marine Online 

Use your IMR Report to determine items that are due.  

“Due” items (Dental, lab, etc.) must be completed prior to your PHA.  

If you are dental Class 3 or 4, you must schedule and go to a dental appointment to complete requirements. 

Step 2. Complete the online PHA  https://eha.health.mil/EHA/ 

You can also search online for “PHA Navy”, click “Periodic Health Assessment (PHA)”, and click “Access PHA log in page:” 

Have your 10-digit DOD ID number and five-digit Unit Identification Code on hand. 

Step 3. Call the DHA Virtually Integrated Patient Readiness and Remote Clinic at 844-863-3236 

Call 844-863-3236; Appointment Line is open Monday-Friday 6 a.m. to 7 p.m. U.S. Central Time.– Service members MUST complete the online portion for the appointment line to schedule their appointment. 

Send a message through the MHS GENESIS Patient Portal for any concerns or issues that arise. This includes service members in a special duty status. 

  • Select “Messaging”    
  • Select “Send a message”  
  • In the “To” box, type “NH Yokosuka Military Readiness Clinic” Please include a good phone number
