Physical Exams

For all Active Duty physical exams and medical readiness related requirements you will need to call 243-5349 and schedule an appointment with the Patient Administration Office located on the 3^rd floor of the hospital in room 3B33.

Prior to your scheduled appointment for a military physical exam, complete the DD form 2807-1 to bring with you to your appointment.
Exams will be accomplished in two parts. The first appointment is administrative to complete paperwork, review your medical record, and to order any labs, studies, or evaluations required for the type of exam. All items must be completed before you will be scheduled for your final appointment with a provider for the exam.
Please be advised that you might have more than one appointment with our office before booking your final appointment with a provider to ensure all paperwork is completed with any necessary results.
 
Additional information:

1. Please ensure your Periodic Health Assessment is up to date before scheduling an appointment for an exams or screenings. See our PHA page for more information on completing your PHA.
2. The rest of your IMR should also be up to date.  Please contact our office at 243-5349 for how to address other delinquent IMR components.

1. Screenings (e.g. High-Risk, Arms, Ammunition and Explosives, Marine Special Duty Assignment initial screening, etc.)
2. Separation/ Retirement Physicals (can be started six months from EAOS or terminal leave)
3. Re-enlistment exams
4. Commissioning/General Physical
5. Flight physicals (started here, but exam will be completed by Aviation Medicine Atsugi- can be reached at 264-3198/3964)
6. Undersea Medicine Physicals (i.e Dive, NSW/SO, Sub, Nuclear Field duty).  These will require a unique DD 2807-1
7. Pre-deployment Health Assessments/Post-Deployment Health Reassessments

8. For motor vehicle exams (i.e forklift, explosives handler, explosive vehicle operator) call Occupational Health at 243-8247
